All completed surveys will be entered into a prize draw where the lucky winner will receive US $500 worth of Amazon vouchers or, alternatively, a US $750 donation to a charity of your choice.

This survey will close on 31 May 2023. After that date no entries will be accepted. Only one entry per person. The winner will be selected at random and contacted within two weeks of the closing date. There is no cash alternative to the prize on offer.
